Nominate a Colleague for a 2017 Award *NOMINATIONS NOW CLOSED*

Do you know a family physician in Ontario who goes above and beyond in caring for their patients? Or someone who regularly demonstrates leadership excellence? How about a tireless and committed family practice team who deserves special recognition?

Each year the OCFP’s Awards program celebrates the outstanding skill, knowledge and dedication of family doctors across Ontario. They recognize excellence in community and medical leadership, outstanding contributions in teaching and exceptional teamwork. Help us celebrate these primary care heroes by nominating a colleague for an Ontario College of Family Physicians Award.

Nominations for the 2017 Awards are now open in five categories:

The Regional Family Physician of the Year Award recognizes the overall outstanding contributions of seven OCFP members who consistently provide exemplary care to their patients and are passionately involved in activities that contribute to ongoing improvements and excellence in family medicine.

The Community Teacher of the Year Award celebrates excellence in a community family medicine preceptor for the academic teaching year. Ontario’s family medicine residents, medical school students and any other health-care learners submit nominations. 

The Family Medicine Resident of the Year Award recognizes outstanding clinical, research, leadership and teaching skills in a family medicine resident currently enrolled in a Family medicine residency program in Ontario.

The Family Practice of the Year Award celebrates the accomplishments and hard work of the members of a family practice in delivering comprehensive services and continuity of care for their patients.

Award of Excellence recognizes exceptional achievement in a specific area or project pertaining to the specialty of family medicine in the last 12 to 24 months.

All nominations are due by Friday March 31, 2017 at 5:00 pm.
