These strategies help to measure patient experience, communicate with diverse patients and actively engage and support patients and their families before, during and after office visits. Key changes for patient-centred interactions include the following:
- Respect patient and family values and expressed needs.
- Encourage patients to expand their role in decision-making, health-related behaviours and self-management.
- Communicate with patients in a culturally appropriate manner in a language and at a level that the patient understands.
- Provide self-management support at every visit through goal-setting and action-planning.
- Obtain feedback from patients and families about their healthcare experience and use this information to improve quality.

Quality Improvement: Quality Improvement in Primary Care–Primary Care Patient Experience Survey
Primary care providers can measure their patients’ experiences in their practices with the Primary Care Patient Experience Survey. The survey was developed by Health Quality Ontario in collaboration with the Association of Family Health Teams of Ontario, the Association of Ontario Health Centres, the Ontario College of Family Physicians and the Ontario Medical Association. Understanding patient perspectives can help provide insight into current strengths and opportunities for improvement and inform the practice’s annual Quality Improvement Plan.
