Kidney Wise logoThe KidneyWise Clinical Toolkit is available for primary care providers to support the care of patients with suspected or chronic kidney disease.

Visit kidneywise.ca to access the toolkit and download the mobile app for iPhone and Android. The toolkit includes:

1. An evidence-based Clinical Algorithm that helps identify and manage chronic kidney disease.

2. An Evidence Summary offering further clinical detail regarding the algorithm content.

3.  An Outpatient Nephrology Referral Form outlining scenarios that would require consultation with a nephrologist.

The Ontario Renal Network is funded by the Ontario Ministry of Health and Long-Term Care and is implementing the Ontario Renal Plan II 2015-2019.
