Clinical Tools and Resources for Primary Care

KidneyWise Clinical Toolkit - Ontario Renal Network

Ontario Renal Network logo

The KidneyWise Clinical Toolkit
provides guidance on the identification, detection and management of chronic kidney disease.

The toolkit has three components:

1. A Clinical Algorithm that can be used at the point of care

2. An Evidence Summary offering Primary Care Providers further details regarding the Clinical Algorithm content including references that were used in development of the Toolkit

3.  An Outpatient Nephrology Referral Form, outlining appropriate clinical scenarios that may require providers to request consultation from a nephrologist, as well as the appropriate investigations that should accompany the referral

The Ontario Renal Network updated the KidneyWise Toolkit in 2018 based on new evidence from relevant clinical practice guidelines and feedback from clinical providers.
