Patient Experience Survey - Health Quality Ontario

The Primary Care Patient Experience Survey supports family doctors and other primary care providers in the collection of feedback about their patients’ experiences of care. Standardized and practice-based, the survey allows practitioners to measure and monitor the responses, which can then be used to help inform Quality Improvement Plans, establish benchmarks and ultimately to enhance quality care delivery in their practices.

The survey was developed by Health Quality Ontario in collaboration with the Association of Family Health Teams of Ontario, the Association of Ontario Health Centres, the Ontario College of Family Physicians and the Ontario Medical Association. Additionally, patients, family physicians, nurse practitioners, and primary care teams also contributed to its development.
